Dear colleagues!

Ten years ago the European Academy of Paediatric Dentistry (EAPD), A. I. Evdokimov Moscow State University of Medicine and Dentistry and the Paediatric Section of the Dental Association of Russia held the 1st Russian-European Congress on paediatric dentistry and launched an important project: a large-scale platform for annual events on pediatric dentistry at the international level in Moscow. This collaboration has become an incentive for pediatric dentists to provide better dental care for children, improve professional skills, promote oral health research and innovative practices.
Oral health care of children is an integral part of the National health care project of the Russian Federation. This certainly gives a great impetus not only to a wide range of congress topics touching upon both dental and paediatric issues, but also to upgrading the level of research, analysis, presentation manner and management. It's essential to have feedback after all the congresses to focus the next ones on the latest trends in the field of pediatric dentistry.
As Rector of the University I expect that many young paediatric dentists will take part in the Congress, those who will be able to achieve professional heights and make their creative contribution to the specialty in the future.

Have a successful congress!

Prof. Jack Toumba
Dear Participants of the 7th Russian-European Congress 2019,
It's the seventh time that we are holding a Congress in Moscow and it's a great honour for me to welcome you to this prestigious and very promising event.
The European Academy of Paediatric Dentistry (EAPD) considers this collaboration as a demonstration of our mutual understanding and desire to further strengthen the cooperation between the Russian group and the EAPD. There is a huge potential for interaction between the EAPD and Russian Paediatric dentists.
For the EAPD, which aims to recruit members from as many European Paediatric dentists as possible, the highest level and standards of Paediatric dental health are essential for professional development and it is significant nowadays that European Paediatric dentistry has demonstrated very high growth dynamics. The EAPD supports the integration processes for this development within Europe.
A new agenda is being formed in today's dental health world, the development paradigm is changing and the latest innovations are altering the face of our civilization. In Russia, as in the entire world, the time has come for a paradigm shift in the development of our specialty.
The EAPD consistently promotes advanced knowledge aimed at improving the quality of dental health for the Paediatric population in Europe. The core of the EAPD policy is to strengthen a favourable climate, to put it into practice and expand global competitiveness. At the same time, in a modern world, the degree of a country's competitiveness of Paediatric dental health specialists depends on the quality of human capital, the pace of development of new technologies and investment activity. And here, cooperation with the EAPD and all National groups of the Academy is especially important.
The EAPD is ready to spread the best practices and to exchange our experience in interacting with all European countries.
Our goal is common. It is to promote the sustainability of Paediatric dentistry development throughout Europe. As Paediatric Dentists we should excel in all three of the following: clinical treatment of children, education of students and trainees and conduct research whilst at the same time maintaining our own personal development. We should continue to attract new members and National groups to work together for the promotion of Paediatric Dentistry and to stimulate innovative activity. All these are new opportunities for professional cooperation between the Paediatric dentists of Russia and the EAPD.
I am confident that the lectures and discussions within the framework of the 7th Russian-European Congress will stimulate intellectual discussions and debates for long-term and successful cooperation between the Russian group and the EAPD. May there be a fruitful dialogue!

Prof. Jack Toumba
EAPD President

Vital Pulp Therapy
In the endodontic involvements of young patients, conventional options in therapy such as pulpectomy and canal compaction systems may not contribute to the tooth life prognosis since it is difficult to close the widely open apices and even if possible, the residual thin root walls will result in skeptical longevity of the teeth.
So another treatment planning such as to provoke self-healing power is required. For example, when pulp is alive apexogenesis can be indicated, when pulp is necrosis without infection revascularization can be expected with immature teeth. Even if apices are closed a natural pulp healing called 'transient apical breakdown' may be expected in subluxation injury.
In this presentation, endodontic options for young patients (so-called Vital Pulp Therapy)will be discussed with showing specific cases.

Regenerative Endodontics: a real paradigm shift in treatment?
Advances in the field of material science, stem cell biology, and dental tissue engineering have raised the possibility of using biology-based treatment strategies to regenerate functional dental tissues including dental pulp.
A few approaches have been explored for dental pulp tissue regeneration. The only one that is currently in general clinical use is the "root canal revascularization via blood clotting" simply called "revascularization".
This technique involves intracanal irrigations and the placement of a combination of antibiotics or calcium hydroxide in the canal for a couple of weeks, Followed by instrumentation of the apical tissue in order to create a blood clot in the canal and the placement of cervical barrier and coronal restoration.
In recent years different materials were introduced in order to try and improve the clinical outcome of revascularization.

Pulpitis in primary teeth: evidence-based treatment condepts
Current approach to pulpitis therapy for primary teeth will be discussed in the presentation. Special attention will be paid to the indications and contraindications, advantages and disadvantages, success and failures of pulpitis treatment with various techniques and materials. Comparative analysis of pulpitis treatment outcomes in children will be performed according to the principles of evidence-based medicine.

Modern non-invasive approach of fluorosis and enamel hypoplasia treatment
The report will present the clinical protocols of modern techniques for white and brown spots on enamel removing with microabrasion and composite resin infiltration. Congress participants will learn about the details of these techniques, as well as get ready-made schemes and proven algorithms used for enamel hypoplasia, tooth fluorosis and local enamel demineralization. Success in these clinical situations is provided by an integrated approach, which will be discussed in detail in the report from the position of step-by-step application of microabrasion, infiltration and remineralizing therapy depending on the location and depth of the lesion. The report will be useful to specialists in pediatric dentistry and prevention of dental diseases.

Bones and Teeth – what bone health can tell us about the dentition.
It has been known for centuries that illness and malnutrition can affect skeletal development in growing children. Bone disease in childhood is known to have an impact on developmentdental hard tissues, but the consequences of disturbances of calcium and phosphate metabolism on the dentition are complicated.
The public often think that insufficient calcium in the diet is the cause of dental decay or dental defects. Because dentists work with diseases of hard tissues, we need to understand how calcium, vitamin D, renal function and bone metabolism can influence dental defects and dental diseases.
Rare skeletal dysplasias can also have a negative impact on dental health, and sometimes the dental manifestations can provide valuable clues to aid diagnosis. A series of paediatricbone diseases with dental manifestations will be presented, describing treatment options.

Restaurative Paediatric Dentistry, which is the best material choice and the best economic solution swiss style
While the use of amalgam for the restoration of posterior cavities is declining, different kind of resins are being more often used. From glass-ionomer cement (HV-GIC/RC) processed with resinous coating, to light - chemically - dual cured composites, which material is the best choice for operative paediatric dentistry ? Several aspects such as the cost plays an important role in the choice of material. In that way this presentation will also intergrate an economic dimension and show some financial solutions opportunes swiss style.

Local anaesthesia in paediatric dentistry
Update on pharmacology of drugs and techniques
Local anaesthesia is crucial for adequate pain management in paediatric dentistry.
A differentiated use of the drugs and solutions is recommended.
The general types of local anaesthetic drugs are esters (e.g. benzocaine, tetracaine) and amides (e.g. articaine, bupivacaine, lidocaine, mepivacaine). Esters are metabolised in tissues and blood and should only be used as topical formulations. Amides are metabolized in the liver, have longer plasma half-life than esters and are more potent anaesthetics. Articaine is special because of its ester linkage; the first step of metabolism is therefore inactivation by plasma cholinesterase and then finally biotransformation in the liver. The dose of the local anaesthetic should be calculated prior treatment with respect to the body weight of the patient und the concomitant diseases. Serious complications are intoxication because of relative and absolute overdosage and allergic reactions.
Because of the vasodilating effect of the local anaesthetics that leads to relevant absorption into the circulating blood and enhanced plasma levels vasoconstrictors are added to the dental formulations when ever possible. The gold standard is adrenaline in different concentrations. To reduce side effect the lowest possible dose should be used.
